  • Health professionals such as doctors or nurses who identify young people needing guidance.

Essentially, anyone who comes into contact with young people or families in need of support can use this page to make a referral or connect with us. 

How to refer a young person

To refer a young person or family to Anchor 1:1, please complete our online referral form. You can access this directly through our website or via our dedicated Google Form. We will review your referral promptly and get in touch to provide personalised support.

This comprehensive process ensures that every young person and family receives personalised, safe, and effective support from the very beginning. 

Referral review

We carefully review the referral form to gain a thorough understanding of the young person’s or family’s needs.

Initial contact

A member of our dedicated team will reach out to the family to introduce Anchor 1:1, explain the support services we offer, and address any initial questions they may have

Assessment and planning

We collaborate closely with the young person and their family to identify their specific goals and determine the most suitable support plan, whether that involves mentoring, family support, or early intervention.

Ongoing support

A tailored support plan is put into action, which may include one-to-one sessions, guidance, and regular check-ins to ensure progress and promote overall wellbeing.

Follow-up

We continuously review the progress made, adjust the support plan as needed, and provide feedback to referrers where appropriate.
